The sense that reads your body from the inside — hunger, thirst, heartbeat, and the physical signals underneath every emotion. Here's why it works differently for many of us.

Interoception is your sense of the internal state of your body — hunger, thirst, temperature, a full bladder, a racing heart, muscle tension, and the physical signals that make up emotion. It's sometimes called the eighth sense. It works alongside sight, sound, and the rest, but it points inward instead of out at the world.

For many neurodivergent people this sense runs differently, and that has knock-on effects from missing meals to struggling to name feelings. What is interoception?

Interoception is the perception of signals coming from inside your body. Your brain constantly receives information from your organs, muscles, and internal systems, and interoception is how that information reaches awareness — or doesn't. It's why you notice you're hungry, cold, or anxious.

Researchers separate a few pieces of it: how accurately you detect signals, how aware you are of them, and how you interpret them.1 Those can come apart. You might have signals firing that never reach awareness, or reach it late, or arrive without a clear meaning attached. Understanding interoception as a chain — signal, awareness, interpretation — makes it easier to see where it breaks down for a given person.

What does interoception track?

More than most people realize. Interoception covers the whole internal dashboard, from basic survival cues to the physical roots of mood.

  • Hunger and thirst — the cues that tell you to eat and drink.
  • Temperature — noticing you're too hot or cold and acting on it.
  • Bathroom signals — the build-up that says you need to go, ideally before it's urgent.
  • Heart rate and breath — the racing or tightness that signals exertion or stress.
  • Muscle tension and pain — the physical load your body is carrying right now.
  • Emotional signals — the bodily side of feelings, from a sinking stomach to a warm chest.

When this dashboard reads clearly, you meet needs early and smoothly. When it's fuzzy, needs pile up unnoticed until they force their way in.

Why interoception differs for neurodivergent people

Many autistic and ADHD people have interoception that runs quieter, louder, or less consistent than average. Signals can arrive late, get missed until they're urgent, or feel hard to interpret once they do land. A review of interoception in autism describes exactly this kind of variability.2

In daily life this looks like not noticing hunger until you're shaky and irritable, not registering a full bladder until it's pressing, or missing thirst entirely on a busy day. It's easy to read as carelessness, including by yourself. It isn't. The internal signals are being read differently, which is a wiring difference, not a lapse in effort. Naming it that way tends to lift a lot of self-blame.

Interoception and emotion

Emotions aren't only mental events. They're built partly from bodily signals — a tight chest, a fast heart, a sinking gut, a flush of heat. Interoception is how you notice those signals, which means it sits underneath your ability to feel and name what's going on.

When interoception is unclear, emotions can feel vague, delayed, or physically confusing. You might know something's off without being able to say what, or only realize you were angry hours later. The feeling was there in the body; the reading of it was the missing step. This is the mechanism that connects a sensory difference to the experience of not being able to find your own feelings.

The link to alexithymia

Alexithymia is difficulty identifying and describing your own emotions. Interoception is a large part of why it happens. If you can't reliably read the body's signals, the raw material of emotion never fully resolves into something nameable.

This isn't a loose analogy — the research ties them together directly. One well-known study found that impaired interoception tracks with alexithymia specifically, rather than with autism as such.3 In other words, the trouble naming feelings runs through interoception. What helps

Interoceptive awareness can be built with practice. The aim isn't to force the signals louder but to read them earlier and more reliably.

  • Scheduled check-ins. If signals arrive late, don't wait for them. Eat, drink, and take breaks on a clock rather than on cue while you rebuild the reading.
  • Body scans. Brief, regular pauses to notice what the body's doing — where it's tense, warm, hungry, tired. Naming it strengthens the link.
  • Connect signal to need. When you catch a signal, practice pairing it with what it's asking for. Shaky means eat. Tight jaw means a break.
